Your new role

• Provide crisis support and case management assistance to residents within a men's hostel and crisis accommodation properties
• Complete client intake assessments, case plans, referrals, and ongoing support activities
• Assist residents with daily living skills, including household tasks, routines, and independent living goals
• Maintain accurate case notes, documentation, and administrative records
• Liaise with internal teams and external support services to assist clients in securing and maintaining housing
• Support residents to access payments, community services, and other relevant programs
• Work collaboratively within a team-managed caseload environment
• Provide support across a 24/7 roster, including day, afternoon, sleepover, and overnight shifts

What you'll need to succeed

• Certificate III in Community Services, Individual Support, or a related qualification
• Experience in client intake, case management, support planning, and referrals
• Strong administrative and documentation skills
• Understanding of crisis intervention, homelessness, or community services environments
• Flexible availability to work a rotating 24/7 roster, including sleepover and overnight shifts
• Team-oriented approach with the ability to work independently when required
