MedicalJobsAustralia.com This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.

It is a community health centre based position requiring travel across sites and into private residences. This position is required to participate in afternoon, weekend and public holiday shifts.

The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

Manage complex care, build trusted client relationships and deliver evidence-based nursing support that improves health outcomes in the community.

What you'll be doing As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and evidence-based clinical interventions to clients in the community. Working both independently and collaboratively, you will help clients and their families navigate complex health needs while supporting positive health outcomes and continuity of care.

.Conduct holistic nursing assessments and develop individualised care plans

.Coordinate care and provide clinical interventions, monitoring and ongoing support

.Educate and empower clients, families and carers to better manage their health conditions

.Collaborate with multidisciplinary teams to deliver safe, integrated and person-centred care

.Maintain accurate clinical documentation and contribute to quality and safety initiatives

Essential Criteria:

.Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)

3.Demonstrated effective communication skills both written and verbal. Effective interpersonal skills showing initiative and leadership.

4.Demonstrated commitment to professional development, evidence-based practice, and an awareness of changes within the health care environment.

5.Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.

6.Demonstrated ability to work with consumers, families and carers in a time of high levels of family stress.

7.Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training

8.Current Drivers Licence valid within Australia and willingness to use for work purposes.
