Achieve Something Great Join a supportive and experienced Community Health Nursing team where collaboration, professional growth, and high-quality patient care are at the heart of what we do. Working alongside skilled Registered Nurses, Enrolled Nurses, and a multidisciplinary team, this role provides the opportunity to further develop community nursing knowledge and skills with ongoing support from both the Clinical Nurse Educator (CNE) and Nurse Unit Manager (NUM). As a Community Health Nurse, you will play a vital role in delivering care in patients' homes, helping individuals maintain their independence, improve their wellbeing, and avoid unnecessary hospital presentations.

Selection Criteria To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA).

Describe your recent experience as a Registered Nurse, including the clinical setting(s) you have worked in. Provide an example of how you used your clinical assessment and decision-making skills to deliver safe, high-quality patient care.

Provide an example of a time when you worked collaboratively with a patient, their carers, and a multidisciplinary team to achieve a positive outcome. How did you ensure communication was effective and responsive to the patient's needs?

Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
