Maria Clinic is looking for a dedicated Registered to join their collaborative, community-focused team. This role centers on chronic disease management and preventative health, offering the flexibility of either full-time or part-time hours.
Core Responsibilities:
Care Coordination: Develop tailored Care Plans and Team Care Arrangements. Patient Support: Help individuals navigate chronic conditions and adopt healthier lifestyle habits. Preventative Health: Perform routine health assessments and champion proactive wellness initiatives. Team Collaboration: Work hand-in-hand with GPs and multidisciplinary healthcare providers. Administration: Manage the day-to-day scheduling and paperwork tied to chronic care appointments.
What we are looking for:
Credentials: Current AHPRA registration as an RN Experience: Prior experience in Chronic Condition Management (CCM) is preferred. Soft Skills: Exceptional communication, sharp organizational skills, and a warm, proactive bedside manner.
What's on Offer:
A competitive salary that starts from $80,000 per annum plus Superannuation. $5,000 bonus per year of completion of service Visa sponsorship 1 month of accomodation upon relocation to Casino.
