Conduct comprehensive assessments across a diverse, manageable caseload
Develop tailored treatment plans that support mobility, independence, and wellbeing
Collaborate with RNs, Exercise Physiologists, Care Managers, and other Allied Health colleagues
A blend of community visits and working from home
