Your new role We are seeking an experienced Case Manager to deliver high-quality, client-focused support services within a Functional Independence framework. This role involves working directly in the community, conducting assessments, coordinating care, and supporting clients, families, and carers. You will also play a key role in mentoring staff, coordinating services across agencies, and identifying service gaps to improve outcomes. Key Responsibilities

Conduct eligibility assessments and holistic needs assessments in the community Develop and review individualised care plans with clients and carers Coordinate services and referrals across providers Advocate for client needs and ensure rights and responsibilities are upheld Monitor and evaluate service delivery and outcomes Liaise with families, carers, and external service providers Represent clients in case conferences and network meetings Promote collaboration across community and health services Mentor and guide Support Workers Provide guidance on roles, responsibilities, and service delivery Contribute to team meetings and organisational goals Maintain accurate client records and documentation Monitor and manage budgets and service costs Ensure compliance with WHS, legislation, and organisational policies Identify service gaps and contribute to organisational improvements

What you'll need to succeed
To succeed in this role, you will demonstrate the following:

Degree in Social/Behavioural Sciences, Occupational Therapy, or Allied Health OR Diploma in Aged Care, Disability, or Community Services with 3+ years' relevant experience Recent experience (within the last 3 years) in: Aged Care, Disability, or Community Services, case management, service coordination, and conducting holistic client assessments Previous experience supervising, mentoring, or supporting staff Demonstrated experience in budget management and service delivery oversight Strong communication and interpersonal skills Ability to work independently and manage competing priorities Experience working with culturally and linguistically diverse communities Strong report writing and record-keeping skills Ability to interpret and apply policies, legislation, and frameworks Demonstrated team leadership capability in managing complex client needs Understanding of, or ability to quickly acquire, the Aged Care Quality Standards, and Commonwealth Home Support Programme (CHSP) / Support at Home Knowledge of WHS, EEO, and Anti-Discrimination principles Current Queensland Driving Licence and access to a reliable vehicle Current National Police Check (suitable for Aged Care employment)

What you'll get in return In return, you will receive an attractive hourly rate between $51.81 - $55.73 + super, dependent on qualifications and experience. The role offers a full-time 38-hour week on an initial 3-6-month temporary contract, with the potential for extension. Working hours are Monday to Friday, with no weekend requirements.
