Your New Company

Your New Role

As a Local Community Connector (LCC), you will support people with disabilities, their families, and carers to access appropriate services and community support.

Key responsibilities include:

Assisting participants with access and planning processes. Providing information on disability, community, and mainstream supports. Developing and monitoring community connection plans. Conducting participant check-ins and supporting plan changes where required. Building relationships with community organisations and service providers. Supporting community engagement activities and local service delivery. Maintaining accurate records and completing administrative tasks.

What You Will Need to Succeed

Experience in disability, community services, health, human services, or a related field. Strong stakeholder engagement and customer service skills. Experience working with Aboriginal and Torres Strait Islander communities. Ability to manage competing priorities and solve problems effectively. Strong written and verbal communication skills. Experience working independently and as part of a team. Proficiency using Microsoft Office and client management systems. Willingness to obtain an NDIS Worker Screening Check and Working with Children Check.

Desirable:

Knowledge of the disability sector and person-centred service delivery. Experience in regional, rural, or remote communities.

The Logistics

Australian Citizenship required. APS4 and APS5 opportunities available. Hybrid working arrangements. Travel within local communities may be required. Driving licence preferred. Community and participant home visits may form part of the role.
