This position provides secondary level support and care in the community to client's requiring, supportive and palliative care, including end of life.

It is a community health centre based position requiring travel across sites and into private residences. This position is required to participate in occasional public holidays.

Manage complex palliative care, lead clinical excellence and build patient-centred outcomes across community, hospital and home settings.

What you'll be doing As a Clinical Nurse Specialist, you will provide expert clinical care, care coordination and leadership to support patients with complex palliative and supportive care needs. You will work collaboratively across the care continuum to improve patient outcomes, reduce avoidable hospital presentations and support individuals to receive care aligned with their goals and preferences.

.Deliver advanced nursing assessment, care planning and complex symptom management

.Coordinate multidisciplinary care to support seamless patient transitions across services

.Lead evidence-based practice improvements and quality initiatives within the service

.Provide clinical leadership, mentorship and specialist advice to nursing and healthcare teams

.Advocate for patient-centred care that promotes dignity, choice and quality of life

Essential Criteria:

.Current Registration as a Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA)

Selection Criteria:

1.Demonstrated at least three (3) years full time equivalent post registration experience in Palliative Care, Cancer Care Services, Advanced Nursing or clinical experience deemed appropriate by the employer

2.Post graduate qualifications in Palliative Care, Cancer Care or Advanced Nursing

3.Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient-client assessment and care planning

4.Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Policies and National and State Standards related to Palliative Care

5.Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations

6.Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based service

7.Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
