PPP VAN Intake Worker

About the role:

As a PPP VAN Intake Worker, you will be the first point of contact for individuals and families affected by violence, abuse and neglect, providing compassionate, trauma-informed support and intervention. Working within a specialist multidisciplinary team, you will conduct assessments, coordinate care, facilitate referrals and advocate for clients to ensure they receive timely and appropriate services.

Key Responsibilities

Conduct intake, screening, psychosocial assessments and brief interventions for clients experiencing violence, abuse and neglect. Provide trauma-informed crisis assessment, response and support, including information regarding medical and legal options. Coordinate care planning and referrals, working collaboratively with internal and external services to achieve positive client outcomes. Deliver client-focused advocacy, consultation and liaison services to clients, families, health professionals and partner agencies. Maintain accurate clinical records, contribute to data collection and quality improvement activities, and participate in business hours and after-hours crisis response services.

Our ideal candidate will:

Hold an recognised degree as a Social Worker, Registered Nurse, Psychologist or Occupational Therapist with Peak Body Membership
Experience providing trauma-informed assessment, crisis intervention and support to individuals and families affected by violence, abuse, neglect or other complex circumstances Well-developed communication and interpersonal skills, with the ability to build effective relationships and work collaboratively with clients, health professionals and partner agencies A strong commitment to client-centred care, with the ability to assess needs, coordinate referrals and advocate for vulnerable clients in a sensitive and professional manner. A current Class C driver's licence
