OCCUPATION Full Time Support Worker

SALARY $82,500 + SUPERANNUATION

SUMMARY Support Workers are vital careers for vulnerable individuals and their families. All Good Care value quality and compassion and would like our team members to have the skillset as well as caring personality to provide help to our clients. Support Workers provide practical support, such as helping with household tasks, filling out paperwork, administering medication and personal care. They also often provide emotional support, too. We rely on support workers to be a reliable and trustworthy advocate to our clients and maintain constant communication with their managers to ensure the quality standards are achieved.

DUTIES AND RESPONSIBILITIES • Provide home care services including personal hygiene assistance, domestic support, respite care, transport assistance, meal preparation and social support in accordance with individualised care plans • Assist clients and families with daily living activities including showering, dressing, grooming, mobility support and maintaining wellbeing and independence • Support clients and families in accessing community activities, medical appointments, social services and social engagements to encourage social inclusion and community participation • Provide practical and emotional support to vulnerable individuals, families and Culturally and Linguistically Diverse (CALD) clients in a respectful and compassionate manner • Monitor and report changes in clients’ wellbeing, family circumstances and support needs to Case Managers and relevant health professionals • Deliver person-centred care and family support services in accordance with support plans, organisational procedures, NDIS practice standards and client goals • Maintain accurate and confidential client documentation, progress notes, incident reports and care records • Communicate and collaborate with Case Managers, allied health professionals, families, carers and multidisciplinary teams to support positive client outcomes • Provide emotional support, companionship, advocacy and encouragement while promoting client dignity, independence and choice • Assist clients with medication support, paperwork, routine tasks and non-clinical care activities in accordance with organisational procedures • Support clients with disability, dementia, chronic health conditions, post-hospital recovery and age-related care needs through safe and compassionate support practices • Identify and report risks, hazards, safeguarding concerns or changes in client or family circumstances in accordance with organisational policies and reporting procedures • Participate in the continuous improvement of client care, family support services and quality care outcomes • Assist clients and families in developing life skills, routine management and independent living capabilities where appropriate • Liaise with internal staff, support coordinators, external agencies and health professionals regarding client care and family support services as required • Maintain professional boundaries, confidentiality and ethical standards in accordance with organisational policies and duty of care obligations • Comply with workplace health and safety requirements, infection control standards, manual handling procedures and incident reporting obligations • Attend mandatory training, team meetings and professional development activities to maintain competency requirements • Demonstrate cultural awareness, compassion and professionalism when supporting clients and families from Culturally and Linguistically Diverse (CALD) and varied social backgrounds • Ensure all duties are performed in accordance with All Good Care policies, NDIS guidelines, aged care standards and relevant Australian legislation and industry requirements

REQUIRED SKILLS • Minimum certificate III in any one of Aged Care, Home and Community Care and Disability Current Level II • Minimum 1 year relevant experience • Own Registered Vehicle • Record of car maintenance and services • First Aid certificate • NDIS orientation training • Full working rights in Australia • Satisfactory Police Check valid within 3 years • Current C Class Drivers Licence (Qld) with registered vehicle and minimum third party car insurance
