Disabilities Services Officer

Job description

Our Support Coordination services are designed to help participants make the most of their NDIS plans by connecting them with the right services, providers, and resources to achieve their goals.

The successful applicant will assist participants in understanding and implementing their NDIS plans effectively. Our team works closely with individuals to identify their needs, explore available support options, and establish service agreements with providers. We focus on building participants’ capacity to manage their supports independently over time.

Tasks

· Helping participants understand their NDIS plan and funding allocations

· clients and assessing the nature and extent of difficulties

· Connecting with service providers and community resources

· Coordinating different supports to ensure seamless service delivery

· Assisting with problem-solving and overcoming barriers to accessing services

· Encouraging independence and informed decision-making

· Assessing community need and resources for health, welfare, housing, employment Interviewing, training and other facilities and services

· Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions

· Liaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and services

You should be able to demonstrate the following qualifications to be considered:

· Relevant Diploma or higher qualification plus at least one year of work experience in an accredited residential care or disability care program setting.
