Where you'll be working Western Sydney Drug Health Services provides specialised support and treatment for individuals experiencing substance use disorders. You will join a collaborative and compassionate multidisciplinary team committed to delivering evidence based, patient centred and culturally safe care across the community.

What you'll be doing As the Clinical Nurse Consultant Grade 1 - GP Liaison, you will provide advanced clinical expertise and care coordination for patients with substance use disorders while strengthening partnerships between Drug Health Services and primary care providers.

• Deliver advanced assessment, care planning and ongoing clinical management
• Build strong relationships with GPs to support coordinated patient care
• Provide education, guidance and resources to primary healthcare providers
• Advocate for patients and assist with navigating healthcare and support services
• Lead quality improvement initiatives and mentor nursing and clinical staff

Essential Criteria

Qualification: Postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.

Registration: AHPRA

Selection Criteria

At least 5 years full time equivalent post registration experience in Drug & Alcohol or any experience deemed appropriate by the public health organisation.

Demonstrated experience in the assessment and management of patients with substance use disorders. Understanding of trauma-informed care and harm reduction principles

Strong communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team

Experience in providing education and support to healthcare providers, including GPs.

Proven ability to consult, review, and develop, both new and existing policies and procedures within the relevant speciality

Computer skills including word processing, presentation, spread sheet applications and clinical information systems

Ability to work independently and manage a complex caseload and willing to work across the WSLHD Drug Health Services as needed

A valid NSW Driver's license to regularly visit healthcare providers
