This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life. It is a community health centre based position requiring travel across sites and into private residences. This position is required to participate in afternoon, weekend and public holiday shifts. The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

What you'll be doing As a Chronic & Complex Community Registered Nurse, you will provide high-quality nursing assessment, care coordination, clinical intervention and health education to clients and their carers. Working both independently and collaboratively, you will deliver evidence-based care that supports clients to manage their health needs and achieve optimal outcomes.
.Conduct comprehensive nursing assessments and develop individualised care plans
.Coordinate care and provide clinical interventions, monitoring and ongoing support
.Educate and empower clients, families and carers in managing chronic and complex conditions
.Collaborate with multidisciplinary teams to deliver safe, effective and integrated care
.Maintain accurate clinical documentation and contribute to continuous quality improvement initiatives

Essential Criteria:
.Registration: Current Registered Nurse division 1 Australian Health Practitioner Regulation Agency (AHPRA)

2.Demonstrated ability to organise and prioritise work
3.Demonstrated effective communication skills both written and verbal
4.Effective interpersonal skills showing initiative and leadership
5.Ability to work as a member of a multidisciplinary team
6.Commitment to continuing professional development
7.Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
8.Current Drivers Licence valid within Australia and willingness to use for work purposes
