Your new role

As an APS6 Clinical Care Coordinator, you will operate at the heart of a specialised clinical service, delivering high-quality, evidence-informed care coordination to clients presenting with complex mental health and psychosocial needs.

This is a dynamic, client-facing position where no two days are the same. You will work closely with individuals and their families to ensure they can access, engage with and benefit from appropriate mental health care and support services.

Key responsibilities will include:

  • Managing a diverse caseload of complex clients, providing short-term, goal-oriented clinical case management focused on improving engagement in care
  • Conducting comprehensive assessments, including risk assessments, to identify unmet health, social and wellbeing needs
  • Developing and implementing tailored care plans that address barriers to treatment, enhance resilience, and support recovery-oriented outcomes
  • Delivering brief clinical interventions and psychosocial support, such as psychoeducation, motivational interviewing and problem-solving strategies
  • Building strong, trusted relationships with clients to foster engagement and sustained participation in treatment
  • Beyond direct client work, you will play a crucial coordination and system navigation role:
  • Acting as a central liaison between clients and service providers, ensuring seamless referral pathways across government, community and specialist services
  • Partnering with internal clinicians and external stakeholders to deliver a joined-up, multidisciplinary approach to care
  • Supporting clients and families to navigate complex systems, reducing fragmentation and improving continuity of care
  • You will also contribute to the broader service delivery model:
  • Maintaining accurate and compliant client records, care plans and progress notes using clinical systems
  • Participating in case reviews, team meetings and clinical supervision to uphold best practice and continuous learning
  • Supporting program development, quality improvement initiatives and service evaluation activities
  • Contributing to community engagement, outreach and group-based programs where required
  • This role offers significant scope to influence positive outcomes at both the individual client level and system level, ensuring vulnerable clients receive timely, appropriate and effective support.

What you'll need to succeed

  • Tertiary qualifications and registration in one of the following: Psychology, Occupational Therapy, Nursing), or Social Work
  • Strong understanding of Mental Health & Case Management/Care Coordination

  • Competitive hourly rate of $67.24 – $76.36 + Super, depending on experience
